This offer entitles you, following the purchase of DAILIES® AquaComfort Plus® from your optician, to your money back if you are not satisfied with the triple action comfort from the product purchased. The money back will be in the form of a cheque from CIBA VISION, the promoter, and made payable to yourself for the sum you paid to your optician for the product (up to a maximum of 3 month supply). 2. In order to take advantage of this offer you must be a NEW DAILIES AquaComfort Plus® patient who has purchased at least one pack of lenses. You will need to return the cardboard box of the product together with proof of purchase (a sales receipt) showing that you bought the product within the last 30 days, and the completed money back offer voucher to CIBA VISION at the address shown. All claims must be accompanied by a minimum of 75% of the quantity of contact lenses purchased at the normal market price. Your sales receipt will be returned to you with your cheque. 3. The money back offer voucher has a value of £0.001 and is not transferable or assignable. 4. A money back offer voucher can be used once and only one voucher can be used per person in conjunction with this offer. 5. The money back offer voucher is open to residents of the UK and Eire only. 6. The price of the product is subject to variation. 7. CIBA VISION has the right to change these Terms and Conditions at any time. 8. CIBA VISION will not accept liability for any errors or omissions in relation to the administration of this offer. 9. CIBA VISION reserves the right to change or to cancel this promotion without notice in circumstances which CIBA VISION, at its absolute discretion, decides are exceptional. 10. For the purposes of the Data Protection Act 1998, your personal data which you submit to CIBA VISION by way of completing this form will be processed by CIBA VISION and/or agencies working on its behalf for the purpose of administrating this money back offer and to verify these Terms and Conditions have been compiled with only and will not be disclosed to any third party agencies. The money back offer is valid until 31/12/2008 and thereafter expires.
